Metro Health Hospital Foundation Receives Novo Nordisk Grant

The Metro Health Hospital Foundation has received a $14,544 grant from Novo Nordisk in support of Metro Health's Diabetes Outreach Education Program.

WYOMING, MI, August 03, 2012 (Press-News.org) The Metro Health Hospital Foundation has received a $14,544 grant from Novo Nordisk in support of Metro Health's Diabetes Outreach Education Program.

The primary purpose of the Diabetes Outreach Education Program is to teach patients with diabetes how to better manage their disease. The goal of the project is to improve patients' blood glucose control by self-glucose monitoring. The objectives are to help patients with diabetes increase their knowledge about diabetes and change behavior related to diabetes management.

The Novo Nordisk grant will allow Metro Health to hold 208 educational sessions. Each session will consist of approximately two hours of education, for a total of ten hours per patient. Certified diabetes educators who are also registered nurses or registered dietitians will teach all educational sessions.

The sessions will cover:

• Daily self-management
• Factors that influence blood glucose
• Diabetes medications
• Insulin training
• Signs and symptoms that occur with the treatment of hyper- and hypoglycemia
• Self-monitoring of blood glucose
• Risk and prevention of long-term complications
• Dietary guidelines
• Restaurant eating
• Individualized meal plans for each participant
• Exercise

Metro Health expects to serve 1,387 patients with outpatient sessions, a 25 percent increase in those served last year.
