Polaris Health Directions Welcomes Two New Clients

Polaris is pleased to announce its new clients: Sunny Hills Services and The Bridge Health Clinics & Research Centers, Inc.

LANGHORNE, PA, December 17, 2010 (Press-News.org) Polaris Health Directions, a national leader in outcomes management for mental health, is pleased to welcome two new clients, The Bridge Health Clinics & Research Centers, Inc. and Sunny Hills Services.

The Bridge Health Clinics & Research Centers, Inc., based in Wisconsin, will implement Polaris's outcomes management systems for adult mental health (Polaris-MH), chemical dependency (Polaris-CD) and the severely and persistently mentally ill (Polaris-ROMS). These evidence-based solutions will support the organization's clinicians in treatment planning and monitoring by transforming real-time data into actionable strategies to provide the most effective care based on an individual client's needs.

Sunny Hills Services, which serves vulnerable youth and their families in the greater San Francisco area, will implement Polaris-CANS to train staff on the use of the Child and Adolescent Needs and Strengths (CANS) assessment. Polaris-CANS can also support clinical treatment planning and includes a full version of the CANS assessment, along with other empirical indicators.

Polaris looks forward to working with both organizations to help them realize better outcomes at lower costs for the people they serve.
