Hospice of Michigan Receives Grant to Expand Community Outreach Efforts in Kent County

Hospice of Michigan has received a $225,000 grant from the Peter C. & Emajean Cook Foundation to develop a community outreach program to reach the Hispanic population in Kent County.

GRAND RAPIDS, MI, February 09, 2013 (Press-News.org) Hospice of Michigan has received a $225,000 grant from the Peter C. & Emajean Cook Foundation to develop a community outreach program to reach the Hispanic population in Kent County.

HOM is working with the Hispanic Center of Western Michigan to develop the program, which will be led by a full-time, bilingual nurse who will serve as community outreach liaison between

HOM and the Hispanic community. The new liaison will focus on:

Creating opportunities for people to learn about hospice services

Developing and promoting health and wholeness

Establishing community partnerships and awareness building initiatives

The Center is a non-profit community-based organization, primarily serving the needs of the Hispanic community in West Michigan.

"Nation-wide, Hispanics are less likely than Caucasians to use hospice services, although there is evidence that their need for services may actually be greater," said Marcie Hillary, vice president of resource development at HOM. "To help change this situation locally, Hospice of Michigan is developing a program in the heart of Grand Rapids' Hispanic culture that will provide a greater awareness of hospice care and the benefits it provides."

The $225,000 grant will support HOM's community outreach program for a three-year period.

Once the program reaches a consistent census of 16-18 patients, it will be supported financially through Medicare, Medicaid and private insurance company reimbursements. The first step in developing the program will be to hire a community outreach liaison.
