Leading Population Health Solutions Provider Helps CA Community Health Center Reduce Emergency Department Visits by 12 Percent

After implementing i2iTracks, i2i Systems' leading population health management solution, Livingston Medical Group successfully reduces their Emergency Department visits and improves their quality of care.

SANTA ROSA, CA, February 28, 2014 (Press-News.org) i2i Systems, the pioneer and leading provider of population health management and business intelligence solutions, helps Livingston Medical Group (LMG) reduce their Emergency Department (ED) visits by 12 percent, and more.

This tremendous feat was due to Livingston Medical Group's implementation and continued use of i2iTracks-i2i Systems' flagship solution used today by more than 1,000 health centers, clinics, hospitals, and private physician practices in 34 states to proactively care for patient populations, and analyze and improve health outcomes. Livingston Medical Group started using i2iTracks in the beginning of 2010 by focusing on perinatal tracking and progressed into tracking their chronic disease and diabetic patients.

Since one of their primary objectives was to reduce Emergency Department visits, they turned their focus to setting up ED tracking in i2iTracks. By implementing this system, they were able to keep track of patients that were coming into the Emergency Department daily, track the schedule and let the providers know when the patients are there for an ED follow up.

With i2iTracks' ED Tracking system, LMG was able to organize patients into groups, identify which patients were new and send letters to get them in for visits. Through the information they captured in i2iTracks, they could identify how many times their patients came in for visits and the reason for those visits to ensure they provided the appropriate education. This system enabled Livingston's providers to monitor whether they were successful with their outreach to those patients and if they actually came in for their visits rather than going to the Emergency Department. Ultimately, they were able to identify this core group of patients and reduce ED visits by 12 percent, contributing to high savings for the organization and improved quality of care.
